2) Complete this application online, print it, and mail it to the
Park District.
3) Print this blank application, complete it with pen or
typewriter, and mail it to the Park District.
4) Visit Park District
Headquarters to pick up an application.

able to print it if you have a printer installed on your system.)

child attends Hidden Meadows Day Camp. |

Pathways Mileage Log
Print the mileage log to record your mileage and earn
an incentive at the end of the year. View the
map to find 23
different pathways within the community. Each
one begins and ends or passes through one of the
Centerville-Washington Park District parks.

sprayground with your group or daycare
organization. Read the information, print the
request form, and submit the completed form to

volunteers must be at least 14 years of age.
